If your vision or health insurance carrier is not represented above: Call our office for a consultation with our insurance department. Zeeland Vision can bill your insurance for you as an out of network provider in order to serve you more fully.
There are things that you can do prior to receiving services to help us process your claims and to avoid unexpected expenses. Usually, it’s just a matter of calling your medical insurance company to verify coverage and receive authorization for services, if necessary. Following are important steps that you should take before receiving services from Zeeland Vision:
Verify that Zeeland Vision is an “in-network” provider. Zeeland Vision participates in many major health insurance plans. However, you should verify with your insurance company that Zeeland Vision is an approved service provider. If it is not, you may be responsible for paying a larger portion of the charges. Many insurance companies post an Approved Provider List on their websites.
Confirm your insurance coverage for a specific service or procedure. Patients should verify with their insurance company whether specific procedures or services will be covered under their plan, and obtain preauthorization, if required. Some procedures or services may not be covered or may require preauthorization in order for your insurance to pay the claim. If questions arise during your care with Zeeland Vision, we will do our best to aid you in this process.
Provide us with updated and accurate information. It is important that you provide us with updated and accurate insurance information for all the insurance plans under which you are covered at the time of service. If you are covered under more than one insurance plan, it is helpful if you indicate which plan is primary. This assists Zeeland Vision and your insurance companies with coordinating plan benefits.
Payment Options: We accept cash, personal checks, American Express®, MasterCard®, Visa®, Discover®, and Trip-Holland, Zeeland, & Grand Rapids.
Payment Policy: Payment is required at the time of service for medical fees. Payment for materials is needed before ordering contact lens supplies or prescription glasses. In emergencies, some exceptions may be submitted to the billing department in order to establish a plan for payment.
